Professional Nursing

Our highly skilled professional home health care nurses specialize in providing care to our clients in the 7-County Metropolitan Area in and around Minneapolis and St. Paul, Minnesota.

They also work closely with clients’ physicians, family members and caregivers to keep clients well, and prevent illness and injury.

Nursing care duties include:

  • Assessing the client’s condition
  • Developing and implementing a unique care plan
  • Administering complex treatment and medications
  • Working with your pharmacy on any medication changes
  • Updating your primary physician about any health changes/progress
  • Directing other members of the home care team (e.g., licensed practical nurses, home health aides, personal care assistants and homemakers)
  • Monitoring clients’ body functions and health status, including checking vital signs (e.g., temperature, blood pressure, pulse, respiration)
  • Cleaning, observing and redressing wounds
  • Performing injections and other hands-on treatments
  • Checking and helping with catheters and feeding tubes
  • Providing nail, foot and skin care
  • Teaching basic tasks of care to family members

Home Health Aid

Alo Home Health Care’s caregivers provide hands-on care to help clients with the activities of daily living. They also provide friendship, socialization and support—including performing basic chores in the home. Home Health Aide services from Home Health Care, Inc. are available in the 7-County Metropolitan Area.

Our Home Health Aides may:

  1. Assist with medications, and blood pressure and blood sugar monitoring
  2. Assist with activities of daily living such as:
    • Light housekeeping
    • Bathing
    • Grooming
    • Dressing and clothing selection
    • Meal preparation
    • Toileting
    • Transfers and positioning
    • Mobility
    • Accompany clients to doctor and other appointments
    • Pick up medications from pharmacies
  3. Perform home chores such as:
    • Helping with personal errands
    • Washing dishes and putting them away
    • Cleaning, sweeping, mopping, vacuuming and dusting
    • Changing linens and making beds
    • Doing laundry and ironing
    • Cleaning stoves, microwaves and refrigerators
    • Emptying the trash
    • Grocery shopping
    • Assisting with pet care
    • Watering and caring for houseplants
    • Planning meals, shopping for ingredients and preparing future meals
  4. Provide friendship, socialization and support including:
    • Visiting with the client
    • Responding to the client’s requests
    • Helping with correspondence
    • Participating in activities the client enjoys, including community events, games and cards, hobbies and entertainment
    • Reading to the client
    • Renting and playing movies
    • Accompanying the client to religious services
    • Visiting neighbors, friends and relatives
    • Planning outings and accompanying the client to shopping centers, sporting events, club events, plays, concerts, movies and restaurants
    • Monitoring the client’s diet and eating
    • Checking foods in the home for expiration dates
    • Helping the client keep a calendar
    • Arranging appointments and providing appointment reminders
    • Organizing the client’s mail, paying bills, and mailing bills and letters
    • Organizing closets
    • Answering the door, overseeing home deliveries and supervising home maintenance
  5. Assisting with wake-up and nighttime routines:
    • Perform tasks that are delegated and supervised by registered nurses
    • Document care and observations as directed by the client’s plan of care
    • Notify the registered nurse of any changes in the client’s health status
    • Assist with range-of-motion exercises

Home Maker and Companionship Services

Respite Services

Respite care givers provide short-term, non-medical respite services to care for older adults (60 years of age or older) in their own home, allowing family members to have regular breaks from caring for an elderly relative.

Physical Therapy

Speech Therapy

The Speech Therapist for Alo Home Health Care, Inc is responsible for the assessment and evaluation of patient care needs related to treating speech and language disorders, and functional training in communication, swallowing, and cognitive impairments. Based on this assessment and evaluation, the Speech Therapist works to help determine a treatment plan, performs interventions aimed at improving and enhancing the patient’s well-being, and evaluates the patient’s progress.

  • Our in-home speech therapist:
    • Provides services within the scope of practice as defined by the state laws governing the practice of speech therapy, in accordance with the plan of care, and in coordination with other members of the health care team.
    • Evaluates the patient’s level of function by applying diagnostic and prognostic functional ability tests. Assists the physician in the development of the therapy plan of care.
    • Treats patients to communicate effectively by expressing thoughts according to the patient’s condition using acceptable standards of practice.
    • Observes, records, and reports to the supervising nurse and/or physician the patient’s response to treatment and changes in the patient’s condition.
    • Instructs the patient, the family and/or caregiver and other members of the health care team in areas of speech therapy in which they can participate.
    • Visits patient according to plan of treatment, completes a progress note for each visit, and submits progress notes to the agency at least twice per week.
    • Completes all patient evaluations and develops the ST plan of care within state specific guidelines. Reports outcomes of evaluation, goals, and anticipated projected frequency of care to supervising nurse within 24 hours of the evaluation.
    • Participates in staff conferences and committees as necessary.
    • Provides in-services to agency staff upon request of supervising nurse.
    • Sends the supervising nurse and/or the physician a written summary report on patient’s condition at least every 60 days.
